Credit Card Payment Agreement

To ensure there is no lapse in the care of my horse, I hereby authorize Foundation Equine to keep my credit card on file, and to charge any amount accrued at the time of service.

*Credit cards are encrypted for storage.

Invoices will be emailed unless other arrangements are made through the office.

Please note: Any balance greater than 40 days past the invoice date will incur an 18% annual interest charge billed to your account at 1.5% per month.
